SO what tuition do I need?
You shouldn't be aiming to "pass" GP, but to score well in them. By well, I mean B3 and above.
You will need to put in more effort in all your subjects. It's only April right now and you're in J1. As you said, your O level Chemistry is C5, and Maths is B3. Going by that, you can hardly do well at the A levels.
My first suggestion is to try to clarify the concepts you have with your teachers. Start from the basic (Chapter 1), and worked on problems. Read, and do homework/tutorials.
Try to work it out on your own for at least 1-2 months before deciding if you need tuition.
As the above poster (UltimaOnline) posted, 1-to-1 tuition are usually more effective than group tuition, but also more expensive. That said, it is still possible to find good group tuitions (class size of less than 4). Look out for tuition centers in the heartlands; they are typically cheaper than those in big shopping malls.

But do take note that 1-to-1 tuition may not be more effective than group tuition. It really has to depend on the tutor.
If you can afford, seek tuition help early. Like ASAP. But if costs is an issue, do what Spiritanto says, clarify concepts with your teachers and classmates first. Do all your homework yourself, and cut all other play time.
But remember, tuition is not a magic pill. It still requires you to work hard, but you learn the same content from a different perspective, which may make things clearer for you.

I believe you have not internalised understanding & application of maths concepts while doing tutorial questions and revising for school assessments; thus you find it difficult to cope with and excel in the assessments. Another possibility is not recognising the variations of questions that could be set; you may be interested to take a look at an example:

Financially benefitting method:
Hunt/Seek/Spam your Tutorial teachers for after school classes, don't care if you have to wait for them at evening before you can see them.In the meantime check out your school LMS for answers and videos (Which is what I do)
Tuition:
If you need a reliable tuition centre. Try out Aspire, its the one I use to attend for Chem and Math, the teachers there are First-Class Honors degree holders and their really good at clearing up concepts. There's a few outlets in the east in Singapore with one at Bedok Mall and the other at 112 Katong.
