Using tools, the best you can find out is the country. If the country is big enough to have states, you can find out that too. Other than that, you need to be part of CID in order to find out such info. Or join the dark side.

Actually I did a survey last time. Asked the ip adds or "found" out the ip adds of my friends over msn or using other simple tools. Then created a list in excel about each regions. Since I know where my friends stay. and I know a few from each region, I can find out the prefix of the ip add :p There were some discrepancies of course but it was rather accurate =)
