1: Door 17 - Carriage 3 (Hence D = Door, C = Carriage)
2: No. Since CCL is fully underground, there is only the track and the 3rd rail
3: 4 Power notches and 3 Braking Notches. 1 Neutral Notch and of course, 1 Emergency Brake notch
4: Yes. TO in the TO cabin can just open the emergency detrainment ramp since the one in this image says how to get into TO cabin and then from there to get the ramp opened and do the emergency evacuation.

2. Of course they have concrete sleepers. Otherwise what else is going to support the rail track? (I suspect you might be referring to a check rail instead)
4. Yes. Instead of pushing the door downwards (which is train to track evac), simply slide the same door aside to give access to the cab of a coupled train. A metal plate is then placed over the gap where passengers/personnel can walk over to the other train.
