It is best to submit your resignation letter to your boss and cc to his/her boss.
If you have any problems about getting your pay after your one month's notification period is over and you've fulfilled the one month's job minus any outstanding leave due to you, report to MOM.

I hereby submit my resignation with effect from (today's date).
As I have x number of leave due to me, my last day of employment will be

(Cite reason - if you REALLY want to resign). If the true reason is becos of your boss's attitude you cannot say so. He/She can make your last month a living hell and if u leave b4 your one month's notice, then YOU have to pay the company one month. Best to cite a reason that your boss cannot stop you from leaving. For e.g., going for full-time studies; migrating overseas; relative needs your help with his/her biz immediately; need a long break to regroup and destress as recommended by yr doctor
If you do not cite a reason, your boss might think you do not have a plausible reason to leave. Some bosses know it's hard to find workers so they will psycho and psycho their staff to stay.
