Yes, you will definitely need to get the latest updated hospital specialist memo/ report to certify.
The reason being, if the camp MO just anyhow upgrade your PES without any support documents and should anything happens to you, he will get charged for negligence by HQ Medical Corps/ faced disciplinary actions by Singapore Medical Council and may even be suspended/ lost his doctor's practising licence. Will he ever risk that?
Please submit the hospital specialist memo to the camp doctor to assess and process.

he have forgotten that after he ORD, to apply for jobs before he start work, there will be job application form to fill in for declaration and there is also a pre-employment medical checkup just before/ immediately after he sign a working employment contract too.

Physically fit is one thing, medically is another. Most of the time, security required medically. In US diabetic joining SWAT maybe case to case basis maybe that one not so serious type. Do you need insulin everyday?
You need to be medically fit to be a security guard.
Imagine that you forget to eat beakfast one day (cose there is no time), you will faint. Or you are forced to take two shifts, the body also cannot take it. If you eat too sugary food, you will also faint. The blood sugar level goes up and down like a roller coaster.
It is best for you to do an administration job, with regular meals and standard working hours.
Most importantly, you may work alone at night. If you faint no one will be there to save you.
Thereore you will be excused gard duty in NS! :) If you aren't, ask for it!

You can try by going into the army first, then ask the MO to refer for upgrading before you are getting ORD. Wait one and half year then you do a medical review again.

I really salute you wanting an upgrade while others keep thinking how to down grade.
But frankly training inside during BMT is easy, what other training you going to do in combat vocation is very different in what you doing outside.
Anyway, the best person to judge is your own docter monitoring your situation, does your own doc think is ok to request the upgrade? If your own doc think is ok, get him to write a recommendation and you straight away submit to CMPB before enlistment...
