IPPT: For RT, Must It Be Booked/ Done Consecutively? How?

Firstly, I am still on the old IPPT system.
I understand that i must book RT within 1 months from my birthday, and complete it within 5 months from my birthday.
However, I have overseas internship from the 2nd to 4th month. So i can only do my RT on the first and last month.
1. Can I book my RT to be like this? Or must it be consecutive, one week after another?
Also, I have serious asthma now because of my heavy smoking, although I was PES A officer in army.
2. Is it possible to be exempted from RT by getting a 5 months MC (effectively until the RT period is over)?
