How to Write Appeal Letter for Waiver of Damaged Camp Pass?

Hi, few days back I've damaged my camp pass and I need to submit a form which was filled by me and my superior/boss (DXO), my boss wanted to help me to wavier the penalty fee which I had to pay for damaged pass, but he don't know how he should write it to make it sound convincing, anyone can help me with this?

let your boss takes the blame (on paper at least)...
e.g. say the DXO accidentally damage it by bending/disfacing when he placed some hardcover folder on your table where you place the card.
e.g. say the DXO accidentally damage it when he drove over it where you drop the card on the camp car park.

Best way is to say something or someone banged into you while you were wearing it. Eg someone got pushed or was falling down and banged you.
Or something else beyond your control.
Anyway dun worry. Put your pass in a plastic holder it will not appear broken. Before you know it, yearly pass renewal and updating, turn in your old pass for a new one.

Even the pass has broken into two, use tape to tape it.
Come on you are going to ORD soon, it will be useless by then.
I remember my pass, due to wear and tear, even the photo and the plastic sheet have come off.
Who cares. It is just telling people, you are going to ORD soon. Even if you do it nicely, it will worn off automatically. .

But the problem is, the one that was broken is a temp pass, my camp pass is brand new though. How sia?

Never get extra already "Tou-xiao" already.
How much is the penalty fee anyway? If its just a few dollars, pay and forget about everything? Why must ponder until you get extras? -
