Maybe SMRT Buses could consider installing similar coil wires that SBS Transit is using on their fleet on the nuts of the wheel to minimise such incidents.
lucky wheel never come off when bus turning or something jialat....otherwise might be worse case.

seems the travelin speed is slow before it happened.
from picture - some bolt holes r bigger than normal so seems its not a 1 day proceeding or some of the nuts were not even there from day 1
i dun work for sbs or smrt so no idea on their daily pre-operation checks

ok, if the rim bolt holes are bigger than normal . tat is usual case of not torque up
if tat stud on the ground belongs to this bus. it wld be a more serious case than not torque up only

I was viewing from the safety aspect of the incident. How was the vehicle passed after maintenance/repair works?
The famous question, What If ? The bus was travelling full speed on the expressway, with pasengers? The maintenance crew are taking short cuts?
The consequences could be very unfortunate.

even there are mixed bags of equipment for the mechaincs to repair, I suppose this is more to human error than engineering
but gud operators are just as important as gud mechainics
