Chances are, it was surfacing somewhere it wasn't supposed to surface. Perhaps where the traffic is too heavy, and it shouldnt' have surfaced there.

I think our RSN will have a bigger problem with civilian ships as compared to the Japanese due to the heavy civilian shipping and shallower waters in our part of the world. I hope such accidents will never happen to our submarines.

I've heard from some mates of mine that our Navy guys are actually not as disciplined as their Air and Ground counter parts. Whether that affects professionalism or not, I leave it to u guys to decide.
The RSS Courageous incident reminds us that our ships are only as good as the people in it. It doesn't matter how formidable our RSS Formidable is, or how deadly silent our submarines are if the people on board are not serious about their work.
